LATEST UPDATE | DepEd Official School Form 9 (SF9)

The official Department of Education (DepEd) School Form 9 (SF9) templates for Grades 1 to 12 are now finalized and available for SY 2026–2027.

Aligned with the three-term grading system, these updated report card files streamline the preparation of learner progress records. Distinct templates are available for Grade 1, Grade 2, Grade 3, Grades 4–10, and Grades 11–12. Each template includes fields for Terms 1–3 grades, final averages, attendance, remarks, and parent signatures. Additionally, the Grade 1 version includes the Performance and Competency Evaluation (PACE) Form.

Class advisers must use these official files and thoroughly verify all student details, grades, competency ratings, and attendance records before printing and issuing report cards.
